At the Boston Spine Care Group, your needs are our priority and we recognize how important it is for you to be seen in a timely manner. In some cases, patients may require special attention or there may be a medical emergency. Under such circumstances, we will do our best to see you as soon as possible.
For new patients, please arrive 15 minutes prior to your appointment to fill out necessary paperwork. For a more efficient check-in process when you visit us, print the forms below, complete them at home, and bring them with you to your appointment. If for some reason you must cancel your appointment, please give us 24 hours’ notice so we can accommodate your request.
Our patient forms are available in Adobe’s Portable Document Format (PDF). If you do not have the Adobe Acrobat Reader installed on your system, you may download it for free.
