For a more efficient check-in process when you visit us, complete the New Patient Packet at home by:
  1. Filling in the interactive fields
  2. Save the paperwork with your changes to your computer
  3. Email your completed paperwork to kgage@mgb.org within 72 hours of your appointment

Please make sure the packet is completed prior to your visit and bring it to your appointment or email the packet back to eghiozzi@partners.org prior to your visit.  
Our patient forms are available in Adobe’s Portable Document Format (PDF). If you do not have the Adobe Acrobat Reader installed on your system, you may download it for free.
If for some reason you must cancel your appointment, please give us 24 hours’ notice by calling us at 617-831-7250 so we can accommodate your request.
